Partner with interdisciplinary teams to create and implement individualized care plans. Educate patients and families on health management and preventative care. Accurately document all care provided in accordance with compliance standards. Uphold infection control and safety policies at all times. Active RN license in good standing. At least 1 year of recent RN experience (geriatrics, long-term care, or community-based care a plus). Current BLS certification. Excellent interpersonal and clinical skills.
