JOB DESCRIPTION

JOB TITLE : Case Manager

FLSA : Non-Exempt

DEPARTMENT : Supportive Housing

LOCATION : Yonkers, NY

REPORTS TO : Service Coordinator

SALARY : $43,000.00

SUMMARY : Directly provides counseling, case management and other supportive services to meet the needs of residents. Services provided include crisis intervention and behavior management. Proof of COVID-19 vaccine is required.

ESSENTIAL DUTIES & RESPONSIBILITIES :

Responsible for individual caseload of single adults and families

  • Facilitating client identification and enrollment into the program
  • Determine eligibility in accordance with HUD guidelines
  • Conduct orientation for all project participants and complete intakes on each household member
  • Maintain accurate, complete and current files on all clients
  • Maintain all required documentation including progress notes, treatment plans, discharge summaries,

Service Participation, Homeless Management Information Systems (HMIS) and submit all in a timely

manner.

  • Conduct assessment of needs, work with the participants to develop service plans and establish short-
  • term and long-term goals and document all activities in progress notes

  • Coordinate services for the participants to ensure achievement in identified goals
  • Assist participants with activities and tasks identified in their individual service plans
  • Conduct home visits as required
  • Provide Supportive Counseling and crisis intervention; may require some after hour’s follow-up to
  • emergency situations.

  • Responsible for providing linkages to community resources and serve as a liaison with community
  • agencies

  • Refer and connect participants with vocational / employment support services as appropriate
  • Communicates with all collateral agencies to ensure participants compliance with service plan.
  • Assist clients in connecting / maintaining connection to substance abuse and mental health programs
  • Accompany clients on appointments as needed
  • Organized and facilitate client groups
  • Respond to team suggestions and integrates recommendations in treatment plans.
  • Participates in clinical team meetings to discuss individual cases and general client progress.
  • Identifies program needs and makes recommendations.
  • Perform related tasks as assigned.
  • EXPERRIENCE, EDUCATION AND SPECIAL REQUIREMENT :

    Bachelor's degree in Human Services field preferred, with 2 years’ experience working in a similar setting and / or working with the substance abuse, mental health and homeless populations. Bi-lingual Spanish a plus. Ability to work some evenings and occasional weekends. Excellent and demonstrated writing ability and strong oral communication skills. Must have knowledge and skill in the use of computer software applications for drafting documents, data management and tracking. Experience in crisis intervention techniques and strength-based interventions. Must have own car and valid driver’s license. Must be able to travel throughout Westchester County.
