POSITION PURPOSE The Hospice Chaplain provides compassionate spiritual and emotional support to patients and their families navigating end-of-life care.
This role involves delivering personalized spiritual guidance, fostering comfort, and respecting the diverse beliefs and needs of those served, while collaborating with the interdisciplinary team to enhance holistic care.
ORGANIZATION This position reports to the Hospice Administrator.
ESSENTIAL FUNCTIONS Attend start of care visits and assess the emotional and spiritual needs of each patient and their family members.
Attend regular scheduled visits to provide comfort, spiritual guidance and support to patients and their families / caregivers.
Provide spiritual and emotional resources tailored to the patients’ and families’ preferences.
Facilitate religious services, rituals, or ceremonies as requested by patients or families, honoring their cultural and spiritual beliefs.
Document care provided and update patient records in the Electronic Medical Record (EMR) system in accordance with company confidentiality and documentation policies.
Develop and implement individualized spiritual care plans in collaboration with the interdisciplinary team.
Respond promptly to emergent spiritual care needs, including after-hours or weekend support as required.
Attend IDG meetings as an integral part of the interdisciplinary team to contribute insight on patient and family needs, enhancing comprehensive care planning.
Other duties as assigned to support the mission of the hospice team. KNOWLEDGE & EXPERIENCE Masters of Divinity or equivalent degree Ordination by a recognized religious organization, preferred Current Basic Cardiac Life Support (BCLS) required Minimum of 1-2 years of recent experience in hospice, palliative care, or a related field providing spiritual care.
Valid Driver's License and access to a dependable vehicle or public transportation Active car insurance Flexibility to work irregular hours including nights and weekends Medical knowledge, patience, compassion, kindness, critical thinking skills, knowledge of infection control practices, stamina, and physical endurance, observational skills, knowledge of occupational hazards and safety rules, communication skills, leadership, empathy Must be a U.S. citizen; work sponsorship is not available MACHINES & EQUIPMENT The incumbent must be able to effectively operate the current computer system, telephone system, and other office machines such as a copier.
