Position : RN-Case Manager (Travel / Contract)

Responsibilities

Manage patient care plans and coordinate services for optimal outcomes.

Conduct assessments to identify patient needs and develop care strategies.

Achieve performance targets and contribute to team success.

Collaborate with healthcare teams to ensure comprehensive patient support.

Participate in professional development and training opportunities.

Engage in continuous learning to enhance clinical skills and knowledge.

Monitor patient progress and adjust care plans as necessary.

Educate patients and families on care plans and health management.

Document patient interactions and maintain accurate records.

Utilize data to track performance and improve service delivery.

Participate in quality improvement initiatives to enhance patient care.

Provide support and guidance to junior staff and interns.

Experience / Qualifications

Valid RN license in California with case management certification preferred.

Commitment to continuous learning and professional development.

Experience in patient care management and healthcare coordination.

Track record of achieving performance targets and objectives.

Strong assessment skills to identify patient needs effectively.

Ability to collaborate with multidisciplinary healthcare teams.

Strong communication skills for effective patient and team interactions.

Proficient in documenting patient interactions and care plans.

Ability to educate patients and families on health management.

Experience in quality improvement initiatives within healthcare.
