Social Services / Admissions Director

Crestline Nursing Center  is seeking a  Social Service / Admissions Director.  The position must function as a team member, team leader, and supervisor to ensure that work is accomplished, and quality care is delivered, supporting team members and taking direction from the Executive Director.  This will be a dual role at two facilities!

JOB DUTIES & RESPONSIBILITIES :

• Develops and maintains a good working rapport with other direct caregivers to assure their participation and support of all programs / services to benefit the residents’ psychosocial wellbeing.
• Assist in identifying and correcting problem areas and determines the proper approach to any questions.
• Always maintains confidentiality of all resident / family information.
• Maintain proper documentation and perform general duties.
• Perform other related activities as assigned or requested.
• Assist in coordinating the referral process from the identification of the appropriate payer source to the analysis of patient information
• Collaborate with patient or representative, physicians, residents, nursing staff members, rehabilitation therapy staff members and social work staff members to ensure seamless transfer to the facility
• Communicates information effectively to physicians, supervisors, facility administrators, and any other pertinent personnel in a timely manner
• Other duties as assigned by supervisor
