Nurse Practitioner – Primary Care (Contract, Merrillville, IN)

We’re seeking an experienced Nurse Practitioner to join an onsite health and wellness center in Merrillville, IN for a 13-week contract assignment. This role is ideal for an NP who values consistent daytime hours, a supportive clinical environment and a chance to focus on patient-centered primary care.

Assignment Details :

Location : Onsite health center in Merrillville, IN

Contract Dates : October 27, 2025 – January 18, 2026

Length : 13 weeks

Schedule : Monday through Friday, 9 : 00 AM – 4 : 30 PM (30-min meal break)

Setting : Outpatient / primary care, wellness-focused practice

What You’ll Do :

Provide direct patient care for primary and episodic needs

Conduct wellness visits, preventive screenings and health education

Collaborate with a care team focused on accessible, patient-first healthcare

Support continuity of care and appropriate follow-up
