Assessment Nurse Practitioner (Adult and / or Gerontology) delivers high-quality, person-centered primary care in various settings, including home, long-term care, and telehealth.
Our NPs conduct annual health risk assessments and comprehensive visits, evaluating medical history, social factors, and health status to develop and coordinate care plans.
Responsibilities include diagnosing, coding conditions, providing health education, interpreting tests, documenting in EMR, and collaborating with care teams.
Qualifications require a master’s or DNP degree, national certification, active licenses, and clinical experience in geriatrics or adult care. Preferred candidates have 3+ years' experience, familiarity with coding and value-based care, and training in geriatric or outpatient settings.
Skills needed include strong communication, attention to detail, time management, and customer service.
