Catholic Charities of Orange, Sullivan & Ulster, is looking for a full-time Bilingual Case Manager in their Ulster County Case Management program.

The hourly rate for the position is $23.00. / hour.  This position comes with benefits.

Summary : This position provides direct case management services to the needy and disenfranchised in Ulster County. It is expected that this is accomplished through advocacy, providing information, referral, crisis intervention services, and short-term case management. The position will also have a role in the reception area, answering telephones and processing clients for services.

Essential Duties and Responsibilities include the following :

1. Provide crisis intervention to the homeless individuals and families facing housing emergencies.

2. Serve as a primary case manager for the New York State Rental Supplement Program Plan.

3. Conducts thorough needs assessment and develops a service plan to address identified, concrete needs using a short-term intervention model.

4. Maintains current and accurate documentation of all activities and services provided to clients.

5. Process cases in reasonable timeframe, preferably 45 days.

6. Answers telephones and provides information needed.

7. Conduct outreach to different communities and perform data collection activities.

8. Assist with food / kids’ pantry distributions.

9. Develops and maintains working relationships with providers throughout Ulster County.

10. Attend community or service provider meetings.

11. Available for planned activities on the weekend and some after-hours.

12. Participates in regular supervisory sessions with immediate supervisor.

13. Attends Agency’s in-service training.

14. Travel to meet clients at other CCOSU offices.

15. Other duties as assigned by the supervisor or demands of the program.

Position Type and Expected Hours of Work :

This is a full-time position. Days and hours of work are generally 9 : 00 AM to 5 PM Monday – Friday.

Additional hours may be required to meet program deadlines, or client needs.

• Ability to work with the public and / or under potentially stressful conditions.
• Basic understanding client engagement and de-escalation techniques.
• Prepare comprehensive progress notes and service plans.
• Remain stationary at a workstation and use a computer at least 30% of the time.
