POSITION SUMMARY / RESPONSIBILITIES

\nOversight and leadership of clinical management and process improvement programs to promote safe and cost effective use of medications. Service provided for neonatal through adult patients in the hospital subject to Joint Commission, OSHA, Medicare / Medicaid standards and Texas State Board of Pharmacy Rules and Regulations. Disseminates clinical information to Medical and Pharmacy Staff and allied health personnel. Support of Pharmacy and Therapeutics (P&T) and Drug Utilization Evaluation Committees and subcommittees of P&T. Oversight of formulary utilization programs and critical pathway / order set development. Significant interaction with Medical and clinical pharmacy and resident staff and pharmacy information systems staff. Provision of clinical information, including formulary and pathway / guideline through the web and online dissemination to health care professionals. Develops reports to monitor usage and identify trends in ordering high cost medications and guides plans to control costs and usage in accordance with therapeutic and cost effective principles. Conducts surveillance and makes recommendations for cost effective therapeutic guidelines. Addresses and helps resolve medication reimbursement issues. Oversight of clinical specialists, ICU decentralized pharmacists and Informatics pharmacist.

EDUCATION / EXPERIENCE

\nLicensure to practice Pharmacy in the State of Texas is required. A Pharm. D. Degree, post-graduate residency, fellowship, or Master’s Degree is required. Five years additional experience in a clinically oriented hospital pharmacy is required. Additionally, certification via BCPS (Board of Pharmaceutical Specialties) is preferred. Expertise in reimbursement is preferred.

LICENSURE / CERTIFICATION

\nMust be licensed by the Texas State Board of Pharmacy.
