Location : Chase Center Rehabilitation and Nursing Services – Logansport, IN

Schedule : Full-Time (35–40 hours / week) & PRN opportunities available

Chase Center Rehabilitation and Nursing Services is seeking a dedicated and results-driven Physical Therapist to join our in-house rehab team. Responsibilities :

Evaluate residents and develop individualized treatment plans based on clinical assessments

Deliver skilled physical therapy services in accordance with professional standards

Set functional goals and track progress using evidence-based methods

Collaborate with interdisciplinary team members to optimize resident outcomes

Educate patients, families, and caregivers on therapy techniques and progress

Document all treatment services accurately and in a timely manner

Qualifications :

Licensed Physical Therapist (PT) in the state of Indiana

Graduate of an accredited Physical Therapy program

Must be at least 18 years of age

Experience in Skilled Nursing Facility (SNF) or Long-Term Care (LTC) preferred

Familiarity with Net Health / Optima a plus
