PURPOSE OF POSITION :
In-Clinic is a privately owned pharmacist collective offering both independent pharmacy, consulting, and clinical services located inside established medical facilities. The Pharmacist role plays a pivotal role in In-Clinic’s patient care and chronic disease management clinical model. This role is dedicated to providing exceptional pharmaceutical care, optimizing patient outcomes, and collaborating with various healthcare professionals to ensure comprehensive patient care.
MAJOR RESPONSIBILITIES & ACCOUNTABILITIES :
- Patient Care Management : Provide direct patient care services, including medication therapy management, medication reconciliation, and comprehensive medication reviews to optimize therapeutic outcomes and enhance patient adherence.
- Chronic Disease Management : Develop and implement individualized care plans for patients with chronic diseases such as diabetes, hypertension, asthma, and cardiovascular diseases, focusing on medication optimization, lifestyle modifications, and patient education.
- Chronic Care Management : Coordinate chronic chare management services for patients with complex medical needs, including monitoring of medication adherence, disease progression, and potential adverse effects.
- Remote Patient Monitoring : Utilize remote patient monitoring technologies to track patient health indicators, medication adherence, and disease progression, and intervene as necessary to prevent complications and improve outcomes.
- Interdisciplinary Collaboration : Collaborate closely with physicians, nurses, dietitians, and other healthcare providers to ensure coordinated and comprehensive care for patients, including participation in multidisciplinary rounds and care conferences.
- Adherence Monitoring : Implement strategies to monitor and improve medication and adherence among patients, such as patient counseling, medication synchronization programs, and adherence assessments.
- Quality Improvement Initiatives : Participate in quality improvement initiatives aimed at enhancing medication safety, optimizing medication use, and achieving performance metrics related to patient outcomes.
- Patient Education : Provide medication counseling, disease education, and lifestyle modification advice to patients and their caregivers to empower them to actively participate in their healthcare management.
QUALIFICATIONS AND EXPERIENCE :
Doctor of Pharmacy (Pharm.D.) degree from and accredited pharmacy program.Active pharmacist licensure in the appropriate state(s).Completion of a PGY-1 (or PGY-2) residency in pharmacy practice or equivalent clinical experience preferred.Board certification in ambulatory care pharmacy (BCAP) or other relevant specialty certification is desirable.Strong clinical knowledge and understanding of chronic disease management principals.Excellent communication, interpersonal, and teamwork skills.Ability to thrive in a fast-paced clinical environment and manage multiple priorities effectively.Proficiency in utilizing electronic health records (EHR) and pharmacy management systems.Commitment to continuous professional development and lifelong learning.PHYSICAL DEMANDS / WORK ENVIRONMENT
Ability to use a keyboard to enter and retrieve data.Must have good eyesight to view computer monitor and phone.Lifting and / or carrying of 5 to 20 lbs.Willing to travelRequirements :
Floater between our Greenville and New Bern facilities. Two days minimum travel required.
