Position Title : Resource Specialist

Work Location : Remote, FL Region B

Duration : 3 months with intent to convert

Work Schedule : 8-5 Mon-Fri

Position Summary :

Walk me through the day to day responsibilities of this the role and a description of the project (Outside of Workday JD) :

• This position will be field based and will provide care management to assist with coordination of care, including assessing for health care needs, referring to health plan providers, providing education on benefits / expanded benefits, and provide community resources to address social determinants of health needs. The caseload is 100 members. This position will document in an electronic health record and will be required to document outreach notes, complete assessments, and complete a care plan on each member.
