Join Our Team as a Mobile Health Services RN Program Coordinator!

The Mobile Health Services RN Program Coordinator position is your chance to lead the charge in delivering essential healthcare services throughout McLean County. In this dynamic role, you will collaborate with Carle and community partners to identify locations, develop policies, and facilitate staffing to launch and grow our Mobile Health Services into a daily operation.

Your Responsibilities

As the Mobile Health Services RN Program Coordinator, you will :

• Coordinate daily nursing activities of the Mobile Health Clinic.
• Ensure all staff members are well-trained and competent in their roles.
• Collaborate with the Mobile Health Services RN Manager on staffing, scheduling, and clinic locations.
• Perform telephone triage for patients of all ages, addressing both acute and chronic needs.
• Develop linkages with chronic disease specialty services for patient referrals.
• Administer immunizations and medications to patients across all age groups.
• Oversee the McLean County MHC ICARE – VFC / CHIP program.
• Coordinate quality oversight of patient throughput, care delivery, and customer service.
• Provide health and wellness education to individuals and groups.
• Act as a preceptor, supporting the development of staff and healthcare students.
• Work closely with the entire mobile health services team to ensure continuity of care.
• Assist in community partnerships to enhance healthcare delivery.
• Collect and interpret data to ensure quality care delivery.
• Serve as the primary contact for community partners at site locations.

Compensation and Benefits

The compensation range for this position is $32.81 - $56.43 per hour , reflecting our commitment to fair and competitive pay based on experience, qualifications, and other factors. Additionally, we offer a comprehensive benefits package to support our team members.
