Job Description :

RN Case manager needed for Fort Worth office.

Monday-Friday, hospice experience is required.

Identify the patient's and family's physical, psychosocial, environmental, safety and developmental needs and individualizing the patient's plan of care based on the needs identified.

Required Qualifications :

1-year recent (within the last 3 years) full-time equivalent experience

Graduate of an accredited nursing program.

License, Certification, Registration

Registered Nurse License (in the state where care is provided)

BLS (AHA)
