The RN Care Manager within the Geisinger at Home program plays a vital role in delivering patient-centered care to Geisinger Health Plan's most complex members in a home-based setting. This position works collaboratively with physicians, nurses, and an interdisciplinary care team to assess, plan, and implement individualized care plans that promote optimal health outcomes.\n\nTypical hours are 4 Shifts : 8 AM to 4 : 30 PM. 1 Shift : 10 AM - 6 : 30 PM\n\nRotation of Weekends & Holidays. Will be fully reviewed at interview.\n\nThis role is to support home visits to patients in Luzerne County.\n\nYour primary territory will be Luzerne County but may need to cover other areas as needed.   • Manage a panel of patients based on geographic location, ensuring continuity and coordination of care.\n

  • Practice at the top of the nursing license, delivering timely interventions at the point of contact to prevent delays in care.\n
  • Engage patients and their support systems in meaningful conversations to align healthcare delivery with patient goals and preferences.\n
  • Delegate appropriate tasks to interdisciplinary team members and maintain strong communication across the care continuum.\n
