The Role : MDS Coordinator We are actively Now Hiring a Full-Time MDS Coordinator. While a Registered Nurse (RN) License is preferred, we will strongly consider a Licensed Practical Nurse (LPN) with relevant MDS experience. Essential Responsibilities The MDS Coordinator is responsible for coordinating the Minimum Data Set (MDS) assessment and care planning process to ensure regulatory compliance and accurate reimbursement. Key duties include : Assessment Management : Monitor, coordinate, and guide the completion of comprehensive resident assessments (MDS) in a timely and accurate manner according to state and federal regulations (RAI manual). Data Entry & Submission : Provide involvement and supervision of inputting MDS assessments into the electronic health record (EHR) system. Compliance & Validation : Transmit MDS assessments to the state and meticulously track and obtain receipt of validation for all submissions. Care Planning : Schedule, facilitate, and actively participate in interdisciplinary care plan conferences on a quarterly, annual, and as-needed basis to develop and revise individualized, goal-directed care plans. Reimbursement Expertise : Ensure optimal and accurate case-mix classification and reimbursement by understanding and applying current Federal Medicare PPS (Prospective Payment System) rules and other state and federal medical reimbursement regulations. Quality Assurance : Collaborate with the Director of Nursing and the interdisciplinary team to use MDS data for quality improvement initiatives. Administrative Duties : Perform all other duties as assigned by the Director of Nursing and Administrator. Qualifications License : Active Registered Nurse (RN) License in Wisconsin is Preferred. Active Licensed Practical Nurse (LPN) License in Wisconsin with demonstrated MDS experience will be considered. Knowledge : Strong working knowledge of Case-Mix, RUGS, the Federal Medicare PPS process, and various state and federal Medical reimbursement mechanisms is essential. Skills : Exceptional attention to detail and organizational skills. Proficiency in utilizing computer systems for MDS completion and documentation (EHR knowledge a plus). Proven ability to work independently while effectively collaborating as a crucial part of the leadership team.
