Hospice Registered Nurse - RN

Company : \nCompassus\n\nPosition Summary\n\nThe Hospice Registered Nurse is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. S / he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The Hospice Registered Nurse functions as an interdisciplinary team (IDT) member to provide routine and emergency assessment, (including admissions responsibilities), educational, and evaluative services to meet the needs of patients and their families. S / he helps the patient and family maintain their maximum level of comfort and coping. S / he flexes schedule to meet agency-staffing needs and provides after-hours emergency support to the patient and family (on-call responsibilities are part of the role requirement).\n\nPosition Specific Responsibilities\n

• Completes routine and emergency assessments on each patient as indicated by departmental policy.\n
• Provides clinical and psycho-social services as indicated by plan of care through case management.\n
• Records observations, treatments, and other pertinent information.\n
• Communicates with IDT, Medical Director, and attending Physician as directed regarding measures to alleviate symptoms, and monitors response to measures implemented.\n
• Provides physical and emotional interventive care to support the patient and family in period of crisis through case management with the psycho-social team.\n
• Prevents unwarranted hospitalizations by meeting patient and family needs in the home and being proactive and reactive during crisis situations.\n
• Case Management of routine and emergency patient care.\n
• Collaborates with IDT to meet needs of patient for clinical and psycho-social interventions.\n
• Collaborates with IDT to coordinate hospice care for the patient and family to ensure appropriateness, continuity, and quality of care.\n
• Updates the POC for IDT.\n
• Communicates and documents changes in the plan of care to the Hospice RNCM or Hospice Director of Clinical Services (DCS).\n
• Assists in identifying the need for intervention of other IDT members.\n
• Effectively communicates patient / family needs to IDT.\n
• Educates patient / caregiver regarding care of patient, disease process, dying process, symptom control, and wound care.\n
• Keeps physician and Medical Director informed of patient needs and condition.\n
• Ensures documentation at bedside.\n
• Notifies other IDT members of physician concerns and suggestions.\n
• Submits appropriate documentation and paper work to contracted facilities at completion of visit.\n
• Completes and submits IDT narratives, time logs, and all required documentation by 10 a.m. of the following day.\n
• Performs other duties as assigned.\n\nEducation and / or Experience\n
• Associate or Bachelor's degree in Nursing required.\n
• Minimum of one (1) year nursing practice or equivalent experience preferred.\n
• Hospice, oncology, home health and / or long-term care experience preferred.\n\nSkills\n
• Mathematical Skills : Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.\n
• Language Skills : Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.\n
• Other Skills and Abilities : Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy.\n\nCertifications, Licenses, and Registrations\n
• Active and unencumbered Registered Nurse license in state(s) of employment required.\n
• Current CPR certification required.\n\nPhysical Demands and Work Environment : The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
