Job Description :

Sunbelt Staffing is seeking an experienced Home Health RN for a 13-week contract in Washington, NC. This role involves providing skilled nursing care to patients in their homes, completing OASIS assessments, and managing individualized care plans. Candidates must be proficient with Homecare Homebase and comfortable working independently in a community setting.

Key Responsibilities :

Conduct skilled nursing visits in patient homes

Complete OASIS start-of-care, recertification, and discharge documentation

Collaborate with physicians and care teams to update care plans

Educate patients and families on treatments and medications

Document all visits and patient interactions using Homecare Homebase

Qualifications :

Active RN license in North Carolina

Minimum 2 years of recent home health nursing experience

Experience with OASIS documentation (required)

Proficiency in Homecare Homebase (required)

BLS certification

Valid drivers license, reliable transportation, and auto insurance

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
