Job Description Outpatient Primary Clinician : Elevate Point is a new thoughtfully designed premier substance use disorder outpatient facility located in downtown Brooklyn. We believe healing is a collaborative journey. At Elevate Point we utilize a combination of evidence-based treatments, innovative interventions, and wellness practices such as yoga and mindfulness. We will offer Outpatient, IOP, PHP and an adolescent program. Elevate Point is licensed by the State of New York’s Office of Addiction Supports and Services and is looking for full-time qualified health professionals designated as Primary Clinicians in the Outpatient Program. Must be a QHP and hold an LMFT and at least 2 years experience in alcoholism and drug treatment. Also possessing a CASAC is strongly preferred. Salary range $70,000-$95,000 commensurate with education and experience. Essential Job Functions Responsibilities include : 1) Hold individual and group counseling sessions; 2) Provide counseling and case management services to clients; 3) Chart patients’ progress, develop patient centered, individualized treatment plans; 4) Complete intakes, level of care determinations, biopsychosocial assessments, and involve family members and collaterals in treatment as appropriate; 5) Participate in multidisciplinary team meetings; 6) Additional duties will be determined by the Program Director and / or Clinical Director; 7) Be an ambassador for RECOVERY! Qualifications and Skills Must have LMFT and at least 2 years experience in alcoholism and drug treatment. Also possessing a CASAC is preferred. SIFI certification is also preferred.
