Nurse Practitioner

Job Title : Advanced Practice Clinician – Nurse Practitioner / Physician Assistant Location : Skagit, Whatcom, and Northern Snohomish Counties, WA Duration : 4–6 months Schedule : Monday–Friday, 8 : 00 AM – 5 : 00 PM Travel : Local travel during shift between multiple facilities Position Summary : Seeking a dedicated Advanced Practice Clinician (APC) to provide integrated, longitudinal care to high-risk members in senior living settings. This role is part of an interdisciplinary team focused on improving outcomes and quality of life for patients with complex medical needs. Primary Responsibilities : Conduct comprehensive clinical assessments Manage acute and chronic medical and behavioral conditions Ensure accurate and complete documentation, including ICD-10 coding Collaborate with interdisciplinary care teams Lead advanced illness and care planning conversations Provide education and counseling to patients and caregivers Required Qualifications : Active and unrestricted license in the state of residence (or ability to obtain by start date) NP : Master of Science in Nursing or Doctor of Nursing Practice from an accredited program PA : Degree from an accredited Physician Assistant program Board certification : NP : AANP or ANCC (FNP, ANP, GNP, or AGACNP) PA : NCCPA Active DEA license / prescriptive authority Ability to meet physical job requirements (e.g., lifting 30 lbs, navigating stairs) Reliable transportation Ability to obtain collaborative practice agreement (if required by state) Fully vaccinated for COVID-19 (per federal, state, and client requirements) Preferred Qualifications : 1 year of clinical experience (preferably in long-term care) Experience with patients with complex behavioral, social, or functional needs Knowledge of geriatrics, chronic illness, and end-of-life care Proficiency in electronic medical records and documentation Pride Global offers eligible employee's comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance and hospital indemnity), 401(k)-retirement savings, life & disability insurance, an employee assistance program, legal support, auto, home insurance, pet insurance and employee discounts with preferred vendors.
