Job Title : Advanced Practice Clinician – Nurse Practitioner / Physician Assistant Location : Skagit, Whatcom, and Northern Snohomish Counties, WA Duration : 4–6 months Schedule : Monday–Friday, 8 : 00 AM – 5 : 00 PM Travel : Local travel during shift between multiple facilities Position Summary : Seeking a dedicated Advanced Practice Clinician (APC) to provide integrated, longitudinal care to high-risk members in senior living settings. This role is part of an interdisciplinary team focused on improving outcomes and quality of life for patients with complex medical needs.
