About the Role\n\nCommunity LIFE is seeking a compassionate and experienced Palliative Care Nurse (PCN) to join our supportive care team. Under the guidance of the Supportive Care Manager, the PCN plays a vital role in developing and enhancing care plans tailored to the unique palliative needs of our elderly participants. This position involves both in-home and center-based care, offering ongoing clinical input and coordination to ensure comfort, dignity, and quality of life.\n\nService Areas : Wilkinsburg, Lower Burrell, Tarentum\nSchedule : Monday – Friday, 7 : 30 AM – 4 : 00 PM (occasional flexibility required based on participant needs)\nMileage reimbursed at the federal rate\nKey Responsibilities\n
- Conduct comprehensive assessments and develop individualized palliative care plans\n
- Provide clinical guidance and support throughout the care delivery process\n
- Collaborate with interdisciplinary teams to ensure holistic, person-centered care\n
- Facilitate end-of-life discussions with participants and families as needed\n
- Maintain accurate documentation and participate in care planning meetings\nQualifications\n\nEducation & Experience : \n
- Graduate of an accredited nursing program\n
- Minimum 2 years of RN experience in palliative or hospice care\n
- Experience in patient education and interdisciplinary collaboration preferred\n\nLicensure & Certifications : \n
- Active Registered Nurse (RN) license in Pennsylvania\n
- Hospice and Palliative Nurse Certification preferred\n
- Valid PA Driver's License required\nSkills & Competencies\n
- Deep understanding of geriatric and end-of-life care needs\n
- Strong clinical knowledge and nursing practice skills\n
- Excellent communication and documentation abilities\n
- Ability to work independently and within a team-based care model\n
- Familiarity with local healthcare and geriatric service networks\n
