The Homemaker / Companion provides homemaking and / or companion services to patients enabling them to remain in the home by supplementing the patient and family / caregiver abilities for home management in accordance with company standards and federal, state, and local standards, guidelines, and regulations.

Essential Activities and Tasks

Care Management and Service Delivery - 80%

• Provides homemaking services, such as bed making, light housework, laundering.
• Plans, shops, prepares, or assists with nutritious meals and prescribed diets, which includes the clean-up of kitchen area.
• Accompanies patient to medical appointments as specified in the care plan.
• Runs errands with patient / family's agreement.
• Provides companionship and socialization for the patient.
• Observes the patient for changes in general physical and mental conditions. Reports any changes noted to the nurse.
• Reminds patients to take their medication as prescribed and scheduled.
• Assists to maintain safety in the home. Notifies nurse of safety concerns.

Quality, Compliance, and Risk Management - 20%

All other duties as assigned.
