Description :

• Meets with each resident face to face at least monthly and as needed
• Weekly vitals -record and monitor for any changes according to physician’s orders
• Review notes and follow up appropriately
• Notify physician and / or family members / MCO of any change of cpndition
• Follow up with residents after falls and incidents
• Process MD orders
• Assist in team meetings
• Coordinate new admissions
• Administer B12 injections
• Check delivery and accuracy of medications, follow up as appropriate
• Coordinate additional health services
• Assess and follow up on skin care
• On-call rotation- answering medical questions, emergencies, and staff support
• Performs all necessary LPN’s duties as required by state regulations.
• Support staff

Other duties as needed

Requirements :

Assisted Living experience

Licensed Practical Nurse License in good standing
