Your Role as a Home Health Physical Therapist :

Here’s a snapshot of what you’ll be doing :

Deliver home-based physical therapy to patients recovering mobility, strength, and functionality after illness or injury.

Collaborate with a dedicated home health team, including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, to focus on patient care and goals.

Observe movements, conduct assessments, document progress, and adjust care plans as necessary.

Guide patients through exercises and hands-on therapy to alleviate pain and enhance mobility.

Educate patients about their care plans, established goals, exercises, and self-care techniques.

Support families by coaching them on the care plan, therapy goals, and safe exercise assistance.

What We’re Looking For :

A few must-haves for our Home Health Physical Therapists :

A graduate of an accredited Physical Therapist Program with an active PT license in Michigan.

A minimum of 1 year of physical therapy experience, ideally in home healthcare within the last 5 years.

Current CPR certification.

Familiarity with state and federal home health regulations.

Strong clinical judgment, excellent interpersonal skills, dexterity, and a compassionate approach.
