MedAdventures is seeking a local contract Physical Therapist for a local contract job in Albuquerque, New Mexico.

Physical Therapist – PACE Program

Location : Albuquerque, NM

Schedule : Full-time

Setting : Community-based senior care

Overview :

Join a mission-driven team dedicated to providing comprehensive care for older adults. We're hiring a Physical Therapist to support participants in a PACE (Program of All-Inclusive Care for the Elderly) setting. This is a meaningful opportunity to focus on functional outcomes and quality of life, not productivity metrics.

Responsibilities :

  • Provide physical therapy services in the PACE center, participant homes, and other community settings
  • Conduct evaluations and develop individualized treatment plans
  • Collaborate with an interdisciplinary team to coordinate care
  • Emphasize fall prevention, functional mobility, and long-term independence
  • Participate in care planning and team meetings to support participant goals

    • Qualifications :

  • Active New Mexico Physical Therapist license in good standing
  • Minimum 1 year of experience, ideally with older adults or in a community-based setting
  • Excellent interpersonal and communication skills
  • Comfortable performing home visits and adapting care to real-world settings
