Assure patient safety by executing appropriate policies and procedures. Provide support for pain management using both pharmacological and non-pharmacological measures, assisting patients and families in identifying individualized comfort measures. Ensure handoff communication includes a report on situations most likely to develop and the problems awaiting the patient. Identify proactively issues related to patient education for medication management and provide discharge planning, including instructions on discharge medications. Explore multiple aspects of care regimens with case managers and healthcare providers to promote appropriate resolution.
