• This is a full-time position, starting at 30 hours per week. It requires availability to work alternating weekends and will require floating to other nearby locations
• \n\nAs a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN), you will utilize advanced clinical expertise to deliver high-quality, evidence-based patient care. Responsibilities include assessing patient needs, administering treatments and medications, collaborating with interdisciplinary teams, and advocating for patients. Your critical thinking and clinical knowledge will help support improved patient outcomes and enhance healthcare delivery.\n\nIn this role, you will : \n
• Administer oral, injectable, intravenous, and topical medications.\n
• Evaluate complex patient conditions, monitor pain levels, and document treatment responses.\n
• Apply advanced knowledge of vital sign monitoring to identify and record abnormalities in blood pressure, heart rate, respiratory rate, and temperature.\n
• Perform routine assessments to monitor patient health and ensure overall well-being.\n
• Manage patient access to healthcare and connect individuals with community resources for medication administration, self-care techniques, and disease management.\n
• Conduct medication reviews to identify potential side effects and drug interactions.\n
• Develop and administer mental health assessments to screen for depression and evaluate mood, behavior, and cognitive function.\n
• Accurately document patient medical history—including illnesses, medications, allergies, surgeries, and family history—using electronic health records.\n
• Use your excellent communication skills to interact with patients, families and other healthcare professionals\n\nTo be considered for this important opportunity to care for patients in the community, you must meet the following qualifications : \n
• Active and in good standing state LVN or LPN license.\n
• Successful completion of the NCLEX-PN (National Council Licensure Examination for Practical Nurses).\n
• Knowledge of medical terminology and clinical documentation standards.\n
• Experience with phlebotomy, vaccine administration, and point-of-care testing.\n
• Excellent communication and organizational skills.\n
• Experience working in a clinical setting with diverse cultural dynamics among staff and patients.\n
• Proven ability to manage sensitive information ethically and responsibly.\n
• Strong problem-solving, critical thinking, and decision-making skills.\n
• Proficiency with electronic medical record (EMR) systems; Epic experience preferred.\n\nEducational Qualifications : \n
• High school diploma or GED\n
• Graduate of a State approved Practical or Vocational nursing program.\n
• Successful completion of the NCLEX-PN (National Council Licensure Examination for Practical Nurses.\n\nPay Range\n\nThe typical pay range for this role is : \n\n$22.40 - $48.67\n\nThis pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.\n\nOur people fuel our future.
