Your Role

As a Family Practice Physician , you will provide comprehensive primary, preventive, and episodic care for both pediatric and adult patients. You will work within a Patient-Centered Medical Home (PCMH) model, collaborating with behavioral health and social service teams to address the full spectrum of health needs—physical, emotional, and social.

Key Responsibilities

• Deliver holistic care to pediatric and adult patients.
• Collaborate with a dynamic care team to promote well-being.
• Utilize integrated support from in-office Behavioral Health and Social Service teams.
