About The Position

Description :
Effectively fills out and completes OASIS documentation in accordance with policies and procedures and regulations.
Demonstrates knowledge of and keeps up-to-date on changes to OASIS documentation requirements as needed.
Ensures documentation is completed in accordance with practice and timeliness.
Utilizes the nursing process to assess patient / caregiver needs in order to provide quality care to assigned patients.
Develops and communicates a realistic plan of care for each patient in accordance with evidence-based practice and regulations.
Includes patients and caregivers in the care plan process.
Serves as Case Manager for assigned patients and actively manages caseload and visit times.
Implements medical plans of care, nursing interventions, and procedures as necessary.
Ensures safe and effective quality of care for patients.
Stays up-to-date on high-tech skills and maintains infection control standards.
Works collaboratively with LPNs to ensure that the plan of care is followed.
Delegates tasks and ensures patient needs are met.
Consistently reviews documentation and acts on recommendations.
Evaluates various aspects of the implemented plan of care, including identified problems and patient responses.
Maintains up-to-date and accurate documentation of nursing care provided.
Ensures effective integration of information for use by the healthcare team.
Follows protocol with processes related to documentation and visit compliance.
Performs patient and caregiver education on medication side effects, treatment options, and discharge instructions.
Identifies inter-disciplinary needs of the patient and communicates documented information to other disciplines.
Accurately fills out agency and regulatory forms in a timely manner.
Carries out visits in a time-effective manner.
Ensures that supervisory visits are not combined with routine visits as necessary.
Assesses patient needs for services and provides ongoing supervision in accordance with regulations.
Transmits patient incident reports to the appropriate specialist in a timely manner.
Coordinates safe patient discharges and ensures all paperwork is processed according to policies and regulations.
Coordinates patient care with physicians, providers, and team members to ensure safe and effective care.
Participates in monthly peer review audits and provides constructive feedback.
Proactively schedules and facilitates case conferences with team members.
Advocates for patient care issues to ensure quality and type of care is sensitive to each patient's needs.
