Job Summary :

A Home Health Care Nurse provides personalized nursing care to patients in their homes, ensuring the delivery of quality care and support in a comfortable and familiar environment. These nurses are responsible for assessing patients' health conditions, administering treatments and medications, monitoring vital signs, and educating patients and their families on managing chronic conditions, medications, and overall health. They collaborate with physicians and other healthcare professionals to develop and implement individualized care plans, ensuring that patients receive the appropriate level of care. Home Health Care Nurses may assist with wound care, rehabilitation, and other specialized medical treatments, while also offering emotional support to both patients and families.

Requirements :

• Familiarity with home health, safety and sanitation standards and procedures
• Understanding of confidentiality and Privacy Laws and nursing best practices
• Adequate knowledge of medical terminology
• Ability to learn electronic verification charting software
• Ability to turn and reposition patients
• Excellent communication and interpersonal skills
• Compassionate and able to handle tough situations as needed
• Valid LPN / RN License
• Provide daily basic care
• Administer medications
• Trach care / suctioning (as needed)
• G-tube feeds (as needed)
• O2 administration (as needed)
• Documentation
• Other duties as assigned
• 2+ years experience is a bonus!
