Job Description

📌 Workload & Expectations

  • Average Visits : 6–7 per day (depending on type of visit)
  • Overtime : Must be pre-approved
  • Coverage Area : Primarily Liberty & Chambers counties, but may float within ~60-mile radius from the facility

📌 Requirements

Mandatory :

  • Home Health experience
  • OASIS experience (non-negotiable)

    • Preferred :

  • Homecare Homebase (HCHB) documentation experience

    • Flex Option :

  • Client may accept candidates without HCHB experience at a reduced bill rate for the first 2 weeks, if manager is willing to train
