Key Responsibilities
Interact with patients via verbal and manual communication, maintaining communication records
Manage call schedules for patients at risk for hospital readmission and inform case managers as needed
Investigate and resolve questions and concerns while ensuring compliance with policies and procedures
Required Qualifications
6 months experience in healthcare, preferably with telephonic patient interaction
High School graduate or equivalent
Basic knowledge of medical terminology
Strong computer software and data entry skills
