Hourly Pay Range :

$20.69 - $30.00 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Position Highlights :

• Position : Care Coordinator Mental Health
• Location : LOH- Naperville
• Full Time 36 hrs / week
• Hours : 9a-5 : 30p weekdays; 8 : 30a-5p weekends

A Brief Overview :

Provides care coordination and assists with discharge planning services to patients who are in need of ongoing behavioral health consults in the EEH emergency departments or behavioral health inpatient care following discharge from the emergency department. Coordinates Psychiatrist consults and discharge plan at the direction of the patient's treatment team. Obtains appropriate releases, communicates with other care providers / hospitals and collaborates with patients and families and the treatment team to arrange and document a comprehensive discharge plan.

Demonstrates the knowledge and skills necessary to provide care appropriate to the age of patients served. This includes knowledge of the physical, social and psychological needs of patients served and the ability to respond appropriately to those needs.
