Overview :

Sentara Health is seeking to hire qualified individuals to join our team as Care Transition Coordinators.

Position Status : Full Time - Day Shift

Position Location : Hampton Roads area in Virginia

Hourly Range : $28.16 / hr - $46.94 / hr

Overview :

Care Transition Coordinator (CTC) serves as the liaison between inpatient providers, discharge planners, and our home-based services. This high-visibility role combines clinical knowledge with strategic sales acumen, playing a critical role in identifying appropriate patients, educating care teams, and influencing discharge decisions to support optimal post-acute transitions. CTC builds and maintains strong relationships with referral sources and educates patients / families on available services. Collaborates with operational teams to ensure timely admissions and optimal patient experience.

The ideal candidate is a results-oriented professional who understands the nuances of referral development, is comfortable navigating acute and post-acute settings, and thrives in a relationship-driven environment.

Education :

High School Diploma (Required)

Certification / Licensure :

RN, LPN, BSW / MSW, LPTA, RT (Required)

Experience :

  • Minimum 1 year of experience in home health, hospice, hospital discharge planning, or care coordination (Required)
  • Proven ability to influence clinical decision-makers and drive referrals
  • Strong communication and interpersonal skills
