The Case Manager is responsible for :
- Assessing the areas of the consumer’s life where assistance is needed.
- Offering and coordinating resources, and services for the person.
- Monitoring service delivery and working with providers to ensure the consumer’s needs are met.
- Provide person centered service planning with person and creating Coordinated Consumer Support Plans.
- Attend regular meetings with the person and their interdisciplinary teams.
- Travel to different sites in different counties for meetings.
- Case managers will be providing case management for people on EW, AC, RULE 185, CADI, CAC, and IDD waivers.
- Other duties as assigned.
Case Manager Requirements :
Applicants must have a valid driver’s license and acceptable driving record as determined by Bestview Care Options policy. Applicants must have a vehicle that is covered with current insurance. Applicant must have a reliable vehicle.Applicants must successfully clear local and / or stated background checks.‘Preferred Qualifications : One to two years’ experience in service coordination or case management is preferred, but not required.Applicants will be required to have internet at home.Applicant must have a 4 year degree in social work, human services, or a related social science degree .DO NOT SHARE ANY PERSONAL INFORMATION VIA PHONE.
