About The Position

Job Title : Speech Language Pathologist

Hours per Shift : 8 : 00 AM - 4 : 30 PM

Experience : Minimum of 1 year

License : Registered with the Board of Registration in Massachusetts

Certifications : None required

Must-Have :

  • Two years of experience in acute care, rehab, or homecare as a Speech Therapist
  • Proficient computer skills with the ability to learn new software applications
  • Experience working in an interdisciplinary team
  • Strong verbal, written, and electronic communication skills
  • Excellent interpersonal and customer service skills

Description :

Provide speech therapy services to patients in their place of residence who have impairments, functional limitations, or disabilities resulting from injury, disease, or other causes.

Patients may range in age from birth to over 100 years old.

Collaborate with patients, families, physicians, and other allied health professionals to identify patient needs and coordinate services to meet those needs.

Manage technical, environmental, and financial resources.

Conduct age-appropriate evaluations that reflect patient history, relevant systems review, and appropriate tests and measures, which are reported to the primary clinician.

Weekend rotations are once every six weeks.

Reassess the plan of treatment at least every two weeks with patients, primary caregivers, and the interdisciplinary team.

Modify and communicate the plan as appropriate to meet discharge goals.

Ensure physician orders are current, consistent with the plan of care, and documented in the medical record.

Submit patient schedules to the rehab scheduler by the end of the day Thursday.

Orient and supervise home health aides in accordance with agency and state guidelines.

Manage patient caseloads in accordance with agency, state, and professional guidelines.

Complete documentation and export it timely according to agency standards.

Update POC devices weekly and as directed.

Complete verification of payor information on admission / evaluation within agency and regulatory guidelines.

Maintain and transmit documentation to support managed care authorizations and DME requirements within designated time frames.

Ensure visit schedules are consistent with authorizations by managed care payors.

Discuss utilization and caseload with the rehab manager on an ongoing basis, making changes toward stated goals.

Ensure reportable outcomes meet agency goals.

Identify realistic, objective, and time-oriented goals for patient performance based on examination findings.

Select treatment and discharge plans based on examination findings and patient goals.

Inform patients of their weekly schedules and any changes as necessary.
