    • Intake Nurse Responsibilities :

  • Serve as a liaison with referral sources, outside agencies, health professionals, and field staff
  • Coordinate incoming calls, providing essential information to patients and referral sources
  • Communicate with patients and families about care / service initiation
  • Conduct intake screenings, documenting and analyzing information for admission decisions
  • Log referrals and prepare reports, providing telephonic support as needed
  • Promote care / services to prospective patients
  • Receive verbal orders from physicians to initiate care / services, where permitted by law
  • Collect and enter customer information into our management information system

    • Essential Requirements :

  • Graduate of an accredited school of nursing
  • Current licensure as a Registered Nurse (RN) in the state of Michigan
  • At least one year of experience as a Registered Nurse (RN)
  • Prior home health care experience is preferred
