Registered Nurse - Behavioral Health (CTI Team) - Bronx, NY

A leading nonprofit supporting individuals and communities through mental health, social services, and recovery programs is seeking a full time Registered Nurse (RN) for their Behavioral Health CTI team. Seeking someone passionate about making a meaningful impact and helping every person reach their full potential.

Location : Bronx, NY (Onsite)

Employment Type : Full Time, Day Shift, Mon-Fri 09 : 00am-05 : 00pm or 08 : 00am-04 : 00pm

Compensation

Salary Range : $92,000 Per Year + 2k for anyone thats Bilingual in Spanish

RN Description

  • Participate in a state-supported program aiding vulnerable individuals during key life changes.
  • Deliver medical and health support within the community, collaborating with a diverse team.
  • Facilitate connections to necessary services and care.
  • Bridge care gaps for participants transitioning from hospital to community.
  • Educate on health, reconcile medications, and guide on disease prevention.
  • Link participants to local resources and healthcare providers.
  • Engage in field visits and offer adaptable, community-centered support.

RN Responsibilities

  • Assess and monitor physical, cognitive, and psychosocial health of participants.
  • Support transitions from hospital / ED to community with care planning and follow-up.
  • Provide health education, medication reconciliation, and disease prevention guidance.
  • Link participants to community resources and healthcare providers.
  • Collaborate with clinicians, care managers, and peer specialists to ensure continuity of care.
  • Conduct field visits and provide flexible, community-based support.
  • RN Qualifications

  • NYS RN license and registration required.
  • Minimum 1 year of post-licensure nursing experience preferred, ideally with behavioral health populations.
  • Strong knowledge of health promotion, disease prevention, and community resources.
  • Ability to work flexibly in field-based settings.
