Summary Seeking full-time / part-time Behavioral Health Therapist to provide home and community based services to individuals in their natural environment.
Paid training is available.
ESSENTIAL DUTIES Provide direct care to clients or families who are assigned to a caseload under clinical supervision of a Clinical Supervisor; Attend and participate in individual or group clinical supervision, treatment planning activities and discharge planning; Function as the client’s advocate in all treatment decisions affecting the client; and Prepare and write accurate progress notes or other documents related to services provided to the client; Assist in treatment plan updates and progress summaries associated with the client’s treatment; Provides appropriate interventions as part of the prescribed treatment Attends patient treatment team meetings for assigned caseload; Collects data / information and relates findings to necessary agency staff; Attend trainings and meetings as required; Assists in discharge planning LEVEL OF EDUCATION / TRAINING / QUALIFICATIONS Bachelor’s or Master's Degree in Counseling, Social Work, Sociology, Psychology, rehabilitation services, special education, early childhood education, secondary education, criminal justice, family and consumer sciences, or human growth and development; Ability to work independently and manage a flexible schedule, with minimal direct supervision.
Must have excellent organizational and communication skills Ability to communicate orally and verbally Ability to handle multiple tasks and meet critical deadlines for data entry Abide by agency’s policies and procedures (confidentiality, incident reporting, FWA, code of ethics, non-discrimination) Must possess a valid driver license Ability to obtain CPR certification within 45 days if position is offered WORKING CONDITIONS The Behavioral Health Therapist usually work in a community-based environment.
Travel to homes will be required.
