Description :

Resume and license verification also need to be attached.

If these items are not completed and attached, the candidate will be declined.

Covid test required 72 hours prior to start date with results issued 48 hours prior.

State license verification is mandatory.

Resume and three references are required.

County criminal check for the past seven years for all counties lived and worked is necessary.

Federal criminal check for seven years or more is required.

Multi-state criminal search for seven years or more is necessary.

Nationwide sex offender search for seven years or more is required.

HHS / OIG search is mandatory.

GSA search is necessary.

Social security number trace is required.

1-Step PPD or Quantiferon test is mandatory, or proof of positive PPD and chest X-ray.

SAM search is required.

Virginia state police background check is necessary.

Mandated reporter form is required.

Sworn statement must be completed.
