Job Description

Job Title : Registered Nurse I

Job Specialty : Home Health

Job Duration : 13 weeks

Shift : Day 5x8-Hour (08 : 00 - 17 : 00)

Guaranteed Hours : No guaranteed hours

Experience : Minimum 1 full year of adult home health experience or 1 full year of hospice experience

License : Must have a valid RN license in hand

Certifications : CPR, BLS

Must-Have :

Proficiency in HomeCare HomeBase (HCHB) and Oasis, with recent experience within the last 6 months or willingness to complete a retraining course
Reliable transportation, valid driver's license, and agency-required liability insurance
Willingness to travel up to 75 miles from the home clinic

Job Description :

Provide, coordinate, and direct home nursing care according to physician's orders based on agency policies and procedures

Supervise and evaluate home health aides and licensed practical / vocational nurses

Conduct home visits and periodic office meetings

Handle an average of 5-8 patients per day and maintain productivity expectations of 30 points per week

Perform various types of cases including start of care, wound care, and follow-up visits

Ensure all documentation is completed within 24 hours of patient appointments

Open to on-call rotations and weekend work as required by location and profession

Be adaptable to floating to nearby clinics to meet financial needs if patient census is low

Complete all necessary training and maintain proficiency in required systems and processes
