Estrella Health and Rehabilitation Center in Avondale, Az is currently seeking a Full Time / Part Time / PRN Speech Language Pathologist to join a established fun, in-house therapy team. Estrella Health and Rehabilitation Center receives support from Ensign Services who provides a wide range of clinical and operational assistance to over 200 independently owned healthcare operations located throughout the United States. We have been in-house with therapy for many years which gives us a significant edge going into the current payment model changes. Our therapy and nursing teams work collaboratively to ensure outstanding outcomes for our patients. Duties :
- Evaluate patients per Physician referral.
- Develop effective treatment plans and obtain approval for services from referring physician.
- Treat patients according to Plan of Care adhering to facility documentation guidelines.
- Communicate with director of rehab and other health team members regarding patient progress, problems and treatment plans.
- Participate in Utilization Review Meeting and Rehabilitation Conferences.
- Participate in in service training program for other staff in the facility.
- Document all regulatory and clinically required information in the electronic documentation system and maintain patient records according to company policy
- Record treatment charges per facility policy and procedure.
- Instruct patient's families or nursing staff in maintenance program.
- Participate in discharge planning.
- Act as a clinical preceptor for affiliating CFY / RFP Speech Language Pathologist and student programs.
- Comply with the American Speech Language and Hearing Practice Act for the state, and the facility policy and procedures.

Experience / Qualifications :
- Graduate of an accredited Speech Language Pathology Therapy program
Licensed Physical Therapist in the state of Arizona (Able to obtain prior to starting)
Must have a valid fingerprint clearance card issued by the Arizona Department of Public Safety. Must complete fingerprint clearance card application within seven (7) days of hire date. Must present actual card within 90 days of applying for fingerprint clearance card.

- Assess and diagnose speech, language, communication, and swallowing disorders in patients
- Develop and implement individualized treatment plans to address patients' specific needs
- Conduct patient evaluations and monitor progress throughout the treatment process
- Collaborate with other healthcare professionals to ensure comprehensive patient care
- Educate patients and their families on speech therapy techniques and strategies for continued progress
Skills :
- Proficient in conducting patient assessments and developing treatment plans
- In-depth knowledge of anatomy, physiology, and speech therapy techniques
- Experience working with patients in acute care or hospital settings
- Ability to provide patient-centered care and adapt treatment plans as needed
- Strong communication and interpersonal skills to effectively collaborate with healthcare team members
- Ability to work independently and manage a caseload of patients effectively
