Job Title : Registered Nurse (RN)

Job Specialty : Home Health & Hospice

Job Duration : 13 weeks

Shift : 9H Days - Monday to Friday 8 a.m. to 5 p.m. with on-call rotation for evenings / weekends, holiday coverage required

Guaranteed Hours : 40 hours per week

Experience : Minimum of 1 year

License : Required

Certifications : Basic Life Support (BLS)

Must-Have :

Home Health & Hospice experience
Current Driver's License & Auto Insurance

Job Description :

Initial assessment of client and family to determine home health needs, including a complete physical assessment and history of current and previous illnesses.

Regularly re-evaluate client nursing needs using health assessment data to determine nursing diagnosis.

Initiate, develop, and revise care plans based on client status and needs. Establish goals incorporating therapeutic, preventive, and rehabilitative nursing actions.

Include the client and family in the planning process.

Administer medications and treatments as prescribed by the physician.

Provide healthcare instructions to the client following assessment and plan of care.

Identify discharge planning needs and implement them before client discharge.

Act as Case Manager when assigned by a Clinical Supervisor, coordinating client care for the assigned caseload.

Prepare clinical notes and communicate updates to the primary physician when necessary and at least every sixty days.

Communicate with the physician regarding client needs and report any changes in the client's condition, obtaining physician orders as required.

Ensure arrangements for equipment and other necessary items and services.

Instruct, supervise, and evaluate home health aide care provided.

Provide care to many patients, including in homes and nursing homes within a 60-mile radius.
