Premier Medical Staffing Services is seeking a travel nurse RN Case Management for a travel nursing job in Concord, North Carolina.

Job Description & Requirements

  • Shift : 12 hours, days

HIGHLIGHTS :

  • Minimum Years of Experience required - At least 3-5 years in Case Management
  • Must have traveled as an RN-Case Manager, no first-time travelers
  • RN : BSN required
  • BLS required
  • Case Management certification (preferred)
  • EPIC experience is required
  • Possible floating to any unit
  • Orientation will vary based on skillset / comfort level - 2-5 days departmental
  • No Call
  • Holiday Expectations : Travelers will work all holidays unless approved off in contract
  • RTO : Manager approval

    • ADDITIONAL DETAILS :

  • Average census and case load / day - 99% capacity on most days; 14-16 patients dependent on unit assigned

  • Patient Population : Dependent on unit - Adult Medical / Surgical, Cardiac, Ortho and possibly Peds
  • Skilled in placement for SNF / IPR / LTAC / Hospice / Home Health, DME, Behavioral Health needs / DSS (peds), Difficult complex placement, will work routinely with homeless population
  • Care management skills : assessment, care coordination and discharge planning
  • Support within the Department :

    • Director

  • Manager(s)
  • CMO
  • UM team that is very supportive - PA is part of this team
  • Peer CMs to support
  • Social Worker(s)
  • Technology / Equipment :

    • EMR : EPIC - required

  • Rover Chat in EPIC and will carry a phone
  • Careport - referral management (similar to Allscripts) - only preferred experience
  • Weekend rotation : will depend on requisition
  • Scrub Color : Navy Blue
  • Parking : Free
  • Department Culture : very supportive and teamwork oriented; Manager(s) are very communicative with changes; Educators are supportive through change; seasoned staff with lots of knowledge
