job_description.job_card.job_descriptionVisit patients (in homes, hospitals, nursing facilities, etc.) who are referred for h ospice care Explain hospice services, discuss goals and care with the patient and families , answer any questions, and obtain necessary consents and legal documents Obtain medical history, current medications, and symptoms Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare / insurance guidelines Coordinate the start of services (nursing, aides, equipment, medications.)
