Sunbelt Staffing is hiring a Home Health RN for a contract assignment in Lake Zurich, IL. The nurse will deliver high-quality, skilled care to patients in their homes, complete OASIS documentation, and manage patient records using Homecare Homebase (HCHB). This role is perfect for an RN who values autonomy, flexibility, and meaningful patient relationships.

Key Responsibilities :

Conduct admissions, resumptions, recerts, and discharges using OASIS

Provide skilled nursing care and patient / caregiver education in the home setting

Document visits and care plans accurately in Homecare Homebase (HCHB)

Collaborate with physicians and the interdisciplinary team for coordinated care

Maintain compliance with agency standards and Joint Commission requirements

Qualifications :

Active RN license in Illinois

Minimum 2 years of RN experience, including at least 1 year in home health

OASIS and Homecare Homebase (HCHB) proficiency required

Reliable vehicle, drivers license, and valid auto insurance

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
