Nurse Practitioner / Physician Assistant-Joslin Diabetes Center

Job Summary :

Advanced Practice Provider will serves as a Full-Time APP, providing care to patients within the Joslin Diabetes Center with Upstate's Nappi Wellness Institute . The APP will provide adult diabetes management in the outpatient setting.

Responsibilities include :

• Providing expert level clinical care & coordination of care via a full range of services that will improve quality of patient care assessing & evaluating patients.
• Ordering appropriate laboratory testing & diagnostic studies, arranging for referrals, testing, consultation & therapeutic services.
• Maintaining outstanding relationships with families, referring physicians & all members of the health care team.

Minimum Qualifications :

NYS licensed / registered NP or PA required. Master's Degree as a Nurse Practitioner or Physician Assistant & current National Certification consistent to the clinical practice also required. Have a NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire.

CPR-BLS Certification required at the time of appointment.

Preferred Qualifications :

Diabetes Management.

Experience in an Ambulatory setting.

Monday - Friday, Days
