We're Hiring at Conexio!\n\nMental Health Group Home\n\nRegistered Nurse (Full Time)\n\n$68,500 +$2,000 Sign On Bonus\n\nNew Castle, DE\n\nFull-Time 40 hours - M-F 8a-4p\n\nABOUT THE ROLE\n\nConexio is seeking a motivated, compassionate candidate to support the individuals in the capacity of a Registered Nurse. The Registered Nurse (RN) is a member of the clinical staff who provides medical expertise to the multidisciplinary teams responsible for the delivery of effective services to the individuals served to promote their health and wellness. The Nurse is an effective member of the team with experience in providing a variety of health and wellness services to persons with behavioral health challenges, development and intellectual disabilities, including development of comprehensive assessments and treatment plans, delivery of a wide range of clinical and supportive services, and working collaboratively with the treatment team and other service providers, including DDDS and DSAMH. The Nurse utilizes evidenced-based practices, including Motivational Enhancement Therapy, Trauma-Informed Care, and medication assisted treatment, to effectively intervene and support individuals served.\n\nRESPONSIBILITIES\n

  • Facilitates health care service needs of individuals served and ensures that these service needs are addressed.\n
  • Competes all required medical assessments pursuant to best practices, contractual obligations, and licensure requirements.\n
  • Ensures that an individual's health care services adhere to the best practice, licensure requirements, and are consistent with the policies, procedures, and protocols of contractors and payers of services.\n
  • Delivers effective wellness and medical services in accordance with policies and procedures; state and federal regulations; and extant professional, ethical, and legal principles.\n
  • Maintains an effective therapeutic alliance with individuals served.\n
  • Communicates and coordinates with individuals receiving services, healthcare providers, family members or legal guardian regarding individuals' health care needs.\n
  • Abides by state and federal regulations, policies, and procedures related to confidentiality, patients' rights (PM 46), and exposure and infection control.\n
  • Provides after hours on-call support to program staff.\n
  • Encourages staff / clients while demonstrating self-initiative in problem solving.\n
  • Assist in the development of data collection and analysis of the quality improvement projects.\n
  • Ensures all client records are maintained according to agency standards, policies and procedures.\n
  • Engages in professional development. Responsible for keeping current in the field. Completes seminars and / or continuing education programs, as appropriate to update knowledge about addiction and mental health services.\n
  • Acts as liaison with outside agencies\n
  • Prepares and submits routine and special reports, inclusive of accurate data, payroll, etc., to supervisor via computer within specified time frames.\n
  • Participates in the development of quality improvement activities designed to enhance agency and program services\n\nEngages in professional development. Responsible for keeping current in the field by attending formal and agency trainings.\n\nREQUIREMENTS\n
  • Bachelor's Degree in Nursing or equivalent / or Associate degree.\n
  • Delaware Registered Nursing License in good standing or Compact License\n
  • Certification in First Aid and CPR\n
  • Valid driver's license in good standing with at least 3 years minimum continuous driving experience with no serious traffic infractions.\n
  • Experience with electronic health records preferred\n
  • Excellent communication skills and organizational skills\n
  • Excellent ability to work with clients with substance abuse, co-occurring and / or mental health disorders\n
  • Excellent writing skills and record keeping skills\n
  • Detail oriented\n
  • Knowledge of addiction, substance abuse treatment services and counseling skills and theory\n\nADDITIONAL QUALIFICATIONS\n
  • Computer knowledge of MS Office, and other HR applications is a plus, not required.\n
  • Possess strong verbal and written communication skills.\n
  • Must be proactive, organized, and possess great follow-up skills.\n
  • Must have the skill / ability to collaborate, partner with, and provide exceptional customer support to a variety of affiliate partners within the organization.\n
