We are seeking an Outpatient Counselor in Mesa, AZ to provide professional behavioral health services to decrease the impact of behavioral health disorders and other emotional disturbances. ?The primary responsibility is to provide assessment and treatment, which requires the application of specialized knowledge in the area of behavioral health diagnosis and intervention. ?Work is conducted on an independent basis. Duties :
Determines client service needs by assessing individual client's level of functioning, behavioral health symptoms and history
Determine diagnosis and treatment needs ?
Develop individualized client treatment plans and review frequently
At each session, determines the degree of Danger to Self (DTS) behavior or intention of client
At each session, determines the degree of Danger to Others (DTO) behavior or intention of client?
If needed, coordinates the petitioning of a client for a court-ordered evaluation
After each session completes appropriate documentation
Documentation must meet standards and be in compliance set by CPR's Quality Management Department
Coordinate and collaborate with all clinic support staff to assist clients with various needs
Provide and follow-up on referrals when appropriate
Completes discharge plan when clinically appropriate
Other duties as assigned
Qualifications and Education Requirements :
Current, unrestricted license issued by the Arizona Board of Behavioral Health Examiners : LMSW (Licensed Masters Social Worker), LAC (Licensed Associate Counselor).?
Documented post-master's experience for two (2) years, including experience within the past 24 months providing Outpatient counseling services, or an equivalent of experience, skills and education. Appropriate reference checks by letter or telephone as requested. Background check, as required by state.

Pay Details : $50,000.00 to $70,000.00 per year
