Position : Director of Care Transitions

As a Director of Care Transitions, you’ll play a critical role in growing our resident community and serving as a vital connection between hospitals, families, and our facility. You will be the first impression of our care – and a trusted guide for those navigating the transition into long-term care.

Key Responsibilities

  • Cultivate and maintain relationships with hospitals, physicians, case managers, and community partners
  • Interview residents, families, and sponsors to gather admission information
  • Coordinate and assist with the admission and orientation process
  • Maintain accurate bed availability and facility census
  • Prepare and share necessary documentation for insurance authorizations
  • Greet new residents and ensure a welcoming transition into the facility
  • Protect resident confidentiality and ensure compliance with HIPAA regulations
  • Plan and lead facility tours for families and referral sources
  • Maintain marketing materials and promotional brochures
  • Organize special events to promote community engagement and staff morale
  • Qualifications

  • Strong proficiency in Microsoft Office (Word, Excel, Outlook)
  • Ability to build and maintain positive relationships with hospital staff and healthcare professionals
  • Warm, professional demeanor with a high level of patience and empathy
  • Ability to relate to individuals at various levels of maturity and functional capacity
  • Strong communication and organizational skills
